For best practice, we suggest the use of the Consideration Checklist and Tabulation Sheets linked below, before accessing one of the Tracks for Assessment or Equipment Request.
(Track 1) Outright Recommendation
(Track 2) Assistive Technology Assessment
Track 1: If the use of the AT Checklists and Tabulation Sheets sufficiently guides the IEP team in determining an appropriate AT solution, choose this option, and use the form below
Track 2: If the IEP team believes the student would benefit from an AT Assessment (which includes creation of Re-evaluation paperwork in SESIS) follow these steps:
Step 1: Upload the Consideration Checklist as a Document Related to IEP.
Step 2. Request a Re-evaluation on SESIS
(*Your SBST may want to be responsible for Step 2. Please communicate clearly.)
Step 3: Notify SBST that these documents are complete.
Step 4: SBST Creates a PWN in SESIS, which generates legal consent to be signed and uploaded
Step 5: SBST uploads consent into PWN and indicates D75 AT as the type of eval.
Step 6: SBST creates APD in SESIS.
Note: Keep in DRAFT status
Step 7: PROVIDER COMPLETES THE ASSISTIVE TECHNOLOGY EVALUATION(D75) IN SESIS
