Training Request Form

Use this form for IEP or 504 Plan Mandated Assistive Technology only.

This form is to be completed ONLY if the school/CSE requires training by the Center for Assistive Technology (CAT) Team and the AT device is already at the school site. Schools are NOT required to fill out this form if they have the ability to utilize the equipment without training. This form does not imply that the suggested recommendation is mandated. The school/CSE team must make a final decision regarding AT (Assistive Technology) at their IEP/504 plan meeting.

Complete the form upon receipt of ALL AT equipment recommended on the IEP. 

Assistive Technology Supplemental Support Services

AT Supplemental Support Services (ATSSS) is an additional support for students, staff and families following the training upon initial receipt of the student's AT device.
