Assess

We build a detailed understanding of each pupil’s needs through:

• Ongoing teacher assessment and observation

• Discussion with parents and carers

• Pupil voice

• Use of appropriate assessments where needed

• Input from external professionals where appropriate

Plan

Support is carefully planned to meet identified needs:

• Clear outcomes are agreed

• Strategies and interventions are selected

• Adjustments to teaching are identified

• Plans are shared with staff and families

Do

Support is delivered consistently and adaptively:

• High-quality teaching is adapted within the classroom

• Interventions are delivered where appropriate

• Staff respond flexibly to pupils’ needs

• Relationships and nurture remain central

Review

Impact is regularly reviewed to ensure progress:

• Outcomes are evaluated and next steps identified

• Plans are adjusted where needed

• Parents and pupils are involved in the review process

• External professionals contribute where appropriate
