All student athletes must have a current physical on file and all Rank One forms completed to stay eligible for Ridgeview Athletics class and sports
Embedded Files
All physicals are are consider current for one year from the date on the paperwork
Physical can be completed at a doctor's office, a walk in clinic, or physical night at Cedar Ridge
Rank One forms include medical history, parent consent, and UIL expectation agreements
RankOne forms must be completed at the beginning of each school year
