Amateur Minor Athlete Waiver and Release of Liability Form - Coaches print and make copies of this form for all of your wrestlers. This form must be signed by a custodial parent for each wrestler on each team/club.

NOTE: This form is required for any insurance claim. No Signed Form = No Claim!!!!

This form may be electronically signed. The electronic signature is the same as a handwritten signature for the purposes of validity, enforceability, and admissibility.

CYW Coaches Sign-up & Verification Form - All Coaches and Staff must complete this form

Concussion Training (for Head Coach Only) - Complete the online course on concussion training through the NFHS website

Code of Conduct (ALL Team Staff) - All staff must agree to abide by the Code of Conduct

Sexual Abuse Prevention Policy (ALL Team Staff) - All staff must agree to abide by the Sexual Abuse Prevention Policy and complete annual training by watching the “Protecting Youth Athletes from Sexual Abuse” video

Lindsay's Law Training (ALL Team Staff) -

Lindsay’s Law is about Sudden Cardiac Arrest (SCA) in youth athletes. This law went into effect in 2017.
Lindsay’s Law applies to all youth sports organizations for athletes aged 19 years or younger whether or not they pay a fee to participate or are sponsored by a business or nonprofit. This includes club sports, practices, and scrimmages
Lindsay’s Law requires both the youth athlete and parent/guardian to acknowledge receipt of information about Sudden Cardiac Arrest by signing a form
All team staff must complete this training annually.
