Brain Injury Services provide individualized supports to assist the eligible member to remain living in their own homes or community who would otherwise require care in a medical institution.
RISE Ltd. takes the time to understand each person’s needs and interests. We collaborate with the individuals and their families or guardians, case managers, vocational providers and other members of their support teams to develop individualized plans focused on reaching new milestones.
Our Brain Injury program offers personalized services including
24/7 on-call support up to 24 hour staffing
Medication and Health management - Promoting healthy lifestyles, Increasing safety awareness, Attending medical appointments, Taking medications, Understanding and assisting with individualized medical needs
Community integration - Learning to navigate in the community, Community awareness
Management of daily activities - Cooking, Cleaning, Personal cares
Financial Management/Representative Payee
Life skills development - Problem solving skills, Learning self-awareness
Transportation - Transportation to medical appointments, community events and activities
Behavioral - Learning effective coping skills, Conflict resolution, Anger management skills, Managing mental health
Protection and Advocacy - Rights and responsibilities, Protection from exploitation
Social - Recognizing social cues, Interrelationship skills, Developing social skills, Developing friendships

Hourly Services: Hourly Residential Services are provided based on the needs of the individual. Services can be provided from just one hour per calendar quarter up to a several hours per day.
Daily Services: Staff are present 24 hours a day, 7 days a week to provide oversight and supervision for any health and safety needs.
Services take place in Northeast Iowa in the surrounding communities of Elkader and Oelwein
Hourly Brain Injury Services are provided in the individual's home, apartment or family home as well as in the community.
Daily Brain Injury Services take place at our 3 group home locations as well as in the community:
Breezy Summit - 10 bed group home in Elkader
Curtis House - 5 bed group home in Oelwein
Frederick House - 5 bed group home in Oelwein
Members may be eligible for Brain Injury services by meeting the following criteria:
Be determined to have a brain injury diagnosis included in a definitive list identified in 441 Iowa Administrative Code 83.81(249A). The Iowa Medicaid Enterprise, Medical Services Unit will confirm the brain injury diagnosis.
Be determined by the Iowa Medicaid Enterprise, Medical Services Unit, to need a level of care that includes one of the following:
Intermediate care facility for the intellectually disabled (ICF/ID)
Nursing facility (NF)
Skilled nursing facility (SNF)
RISE Ltd. provides Brain Injury services in Hourly and Daily Service settings.
