Students with epinephrine orders, diabetes, seizures and asthma must submit yearly management forms from their health provider. Parents of students with seizures and diabetes please email the nurses directly as your provider may have their own paperwork.
Students with these conditions MUST submit these orders PRIOR TO TRY-OUTS to be approved for sports participation.
Students should have their own inhaler, epinephrine autoinjector, and emergency diabetic supplies with them at all practices and games/clubs.

Los estudiantes con órdenes de epinefrina, diabetes, convulsiones y asma deben presentar formularios de control anuales de su proveedor de salud. Padres de estudiantes con convulsiones y diabetes, envíen un correo electrónico a las enfermeras directamente, ya que su proveedor puede tener su propia documentación.
Los estudiantes con estas condiciones DEBEN enviar estas órdenes ANTES DE LAS PRUEBAS para ser aprobados para participar en deportes.
Los estudiantes deben tener su propio inhalador, autoinyector de epinefrina y suministros de emergencia para diabéticos con ellos en todas las prácticas y juegos/clubes.

Self admin/carry orders for students who may need to use an inhaler after school (when a nurse is not present) are important for athletes/clubs and strongly encouraged .
Carefully read and check appropriate boxes to permit by parent and physician .

As needed Over the counter medication. "for minor ailments (headaches or menstrual cramps) The school nurse may give Advil or Tylenol when this form is filled out by a parent. If a student requires more than two doses of Ibuprofen/Tylenol in a month than the Physician must complete the form.
