Students with epinephrine orders, diabetes, seizures and asthma must submit yearly management forms from their health provider. Parents of students with seizures and diabetes please email the nurses directly as your provider may have their own paperwork.
Students with these conditions MUST submit these orders PRIOR TO TRY-OUTS to be approved for sports participation.
Students should have their own inhaler, epinephrine autoinjector, and emergency diabetic supplies with them at all practices and games/clubs.

As needed Over the counter medication. "for minor ailments (headaches or menstrual cramps) The school nurse may give Advil or Tylenol when this form is filled out by a parent. If a student requires more than two doses of Ibuprofen/Tylenol in a month than the Physician must complete the form.
