Sports Forms Due: - For Fall Sports: Monday,Aug 17th ,2026 2026-2027 School Year

Winter - Forms Due, Tryouts Winter Sports: Wrestling, Cheer, Basketball (girls & boys)

Spring - Forms Due. Tryouts Spring Sports: LAX(girls & boys), track (girls & boys) , baseball , softball

Sports Forms are accepted until the end of the school day 2:35PM on the due date.

🏃‍♂️ Sports Physicals – What to Submit

For student-athletes, only the Preparticipation Physical Evaluation (PPE) Medical Eligibility Form needs to be submitted to the school.

If the physical exam was completed more than 90 days before the start of the season, please also submit the Health History Update Questionnaire.

📄 The full exam packet—including History and Physical Examination forms—is retained by your healthcare provider and does not need to be submitted.
