As you prepare for the 2025–26 school year starting on September 2, 2025, please review the Health Forms section and have the necessary forms completed by your child’s healthcare provider (HCP) before school begins.
Birthdays After October 1: If your child’s birthday is after October 1, please have your HCP complete the physical exam form based on last year’s physical.
Seasonal Allergies: Be sure to note any seasonal allergies on the physical exam form if your child experiences symptoms.
Immunization Records: Please include an up-to-date immunization record along with the physical exam form.
Food Allergies & Epinephrine Pens
If your child has food allergies and requires an epinephrine auto-injector, please have the Epinephrine Administration Form completed by your child’s doctor. On the first day of school, parents must bring two epinephrine pens in their original packaging, along with the completed forms, directly to the nurse’s office.
School meals are provided by Maschio’s, which employs a registered dietitian who can review menus and ingredients with you and assist in creating safe meal options for your child.
If your child has asthma, please have their Asthma Action Plan completed and signed by their primary care physician prior to the start of school. On the first day, parents should bring the inhaler and the signed action plan to the nurse.
The New Jersey Department of Health (NJDOH) requires that all children entering school meet the age-appropriate immunization guidelines outlined in state regulations (N.J.A.C. 8:57-4).
To avoid delays or last-minute appointments, parents and guardians are strongly encouraged to schedule visits with their child’s healthcare provider early—well before the busy back-to-school season in August.
Ensuring your child is up to date with required vaccinations is essential for school attendance and helps keep our entire school community healthy and safe.
All required health forms and medical documentation must be submitted before the start of the school year.
Families transferring from out of state or out of the country have 30 days from the date of enrollment to provide the necessary medical records.
If documentation is not received within this timeframe, your child may be excluded from school until all forms are submitted.
Students who are not up to date on immunizations and do not qualify for provisional admission will not be permitted to attend school until they meet the requirements or submit an approved exemption.
Please note that New Jersey permits medical and religious exemptions when applicable and properly documented.
