A completed and signed Physical Exam Form is required in the following situations:
Entering Kindergarten
Transferring from another New Jersey school district
The physical exam must be current (completed within the last 12 months) and signed by a licensed healthcare provider.
Students transferring from outside New Jersey (either from another state or another country) are allowed 30 days from the date of enrollment to submit the required medical documentation, including:
A completed Physical Exam Form
Up-to-date Immunization Records
Failure to provide this documentation within 30 days may result in exclusion from school until all required forms are received.
An Asthma Action Plan must be submitted annually for all students diagnosed with asthma who require medication, whether taken daily or as needed.
This plan must be:
Completed and signed by your child’s healthcare provider
Submitted to the school nurse prior to the first day of school
Timely submission ensures your child’s safety and allows staff to respond appropriately in the event of an asthma-related concern.
For students with anaphylactic (life-threatening) allergies, the following is required each school year:
A completed and signed Epinephrine Administration Form
Submission of the form to the school nurse before the first day of school
Parents/guardians must also provide the school with epinephrine auto-injectors to be kept on site. These must be available for use during the school day and for any off-campus activities or field trips.
Providing these items ensures your child’s safety and allows trained staff to respond immediately in the event of a severe allergic reaction.
This form authorizes the school nurse to administer medication to a student during the school day, either:
On a daily basis (e.g., medication for ADHD), or
On an as-needed basis (e.g., Advil or Tylenol for headaches/minor pain, or allergy medications such as Claritin or Allegra)
A separate form must be completed for each medication
The form must be filled out by the student’s healthcare provider
It must be signed by a parent or guardian
This form does not apply to asthma medications or epinephrine auto-injectors — those require different, specific forms
All medication must be provided in its original labeled container and hand-delivered to the school nurse by a parent or guardian.
A Seizure Action Plan is required for any student with a diagnosed seizure disorder to ensure their safety and proper care while at school.
A completed and signed Seizure Action Plan from your child’s healthcare provider
Submission of the plan to the school nurse prior to the first day of school
Any necessary medications (e.g., rescue medications such as Diastat or intranasal Valtoco or Midazolam) must be provided in their original pharmacy-labeled containers
Medications must be hand-delivered to the school nurse by a parent or guardian
The Seizure Action Plan helps staff recognize the signs of a seizure and provides step-by-step instructions for how to respond appropriately during and after a seizure, both in school and during school-sponsored activities.
