KINDERGARTEN REQUIREMENTS FOR 

2024-2025 SCHOOL YEAR

Health Forms and Formuaritos de Salus

In preparation for the 2023-24 school year, parents of all in-coming kindergarteners MUST submit a copy of their child's most recent PHYSICAL EXAM (within the last calendar year) and a copy of their IMMUNIZATIONS. Flu vaccine is required for students attending preschool. 

If your child's birthday is after October 1st, please have your pediatrician complete forms based on last years' physical. If your child has seasonal allergies, this should be noted on the physical exam form.

If your child has Asthma or Food Allergies (requiring an Epi-pen), please have the forms, found under Health Forms, completed and signed by their PCP before the start of school. The medication must be hand delivered to the school nurse on the first day of school. If you plan on having your child buy lunch at school, please review the Food Allergy page.

The New Jersey Department of Health (NJDOH), Vaccine Preventable Disease Program is reminding parents and healthcare providers (HCPs) that all children must comply with age-appropriate immunization requirements set forth in N.J.A.C. 8:57-4.  Parents and guardians should plan ahead and schedule appointments with their pediatricians now to avoid the back-to-school rush in August.

Please submit ALL required paperwork  prior to school starting. Any students transferring into district from out of state or county, there is a 30 day period for medical documentation to be submitted. 

 If it is not submitted, then your child may be excluded until the paperwork is turned in.

(Children who are not up to date on their immunization requirements and/or do not meet eligibility for provisional admission, will NOT be allowed to attend school until immunizations are received or applicable exemptions are filed).

New Jersey still allows for medical and religious exemptions when applicable.
