Health Forms and Formuaritos de Salus
In preparation for the upcoming school year, parents/guardians of all incoming kindergarten students must submit the following documentation:
A copy of your child’s most recent physical exam (dated within the last calendar year)
An updated immunization record
For Preschool Students:
A flu vaccine is required for all children attending preschool.
Physical Exams: If your child’s birthday falls after October 1, your pediatrician may complete the required forms based on last year’s physical.
Seasonal Allergies: Be sure to have any seasonal allergies clearly noted on the physical exam form.
Asthma or Food Allergies:
If your child has Asthma or Food Allergies requiring an EpiPen, please have the appropriate forms completed and signed by your child's Primary Care Provider (PCP).

Medications must be hand-delivered to the school nurse on the first day of school.

The New Jersey Department of Health (NJDOH) reminds families that all students must comply with immunization requirements as outlined in N.J.A.C. 8:57-4.
Please schedule appointments early with your child’s pediatrician to avoid delays in August.
Children not up to date on immunizations or without applicable exemptions will not be allowed to attend school until all requirements are met.
New Jersey allows for medical and religious exemptions when applicable.
All required forms and documentation must be submitted before the first day of school.
Students transferring from out of state or county have a 30-day window to provide complete medical documentation.
If not received within this period, the student may be excluded from school until documentation is submitted.
