To help keep all Scouts safe, the BSA Annual Health and Medical Record (Parts A & B) is required for every youth and adult participating in our pack. These forms include basic health information, emergency contacts, and a release for activities.
Part A covers general information and consent.
Part B includes a health history and must be completed by a parent or guardian—no doctor’s signature is required for most pack activities.
We require printed copies of Parts A & B to be submitted each year, typically at the start of the Scouting season or when joining the pack. These forms are kept on file by the pack leadership and are required for participation in meetings, outings, and events.
