Conflict Form

Use this form to document any time you are unable to completely attend a rehearsal, performance, or required event. Please refer to the Choir Handbook for specific information about excused/unexcused absences.

Conflicts must be documented at least 3 weeks prior to known conflict. Work is NOT considered excused. Contact your teacher of record if you need someone to talk to your employer. 

In order to maintain confidentiality, only the STUDENT can complete this form using their SCHOOL EMAIL ADDRESS.
