Closet/Cabinet Sign Secure Storage CampusSecure Storage Identification - Restricted Access for Locked Cabinets or Drawers
Required sign on the locked cabinet or drawer where secure test materials will be stored. Campus coordinator will add name and phone number for contact.
Secure Storage Certification - 1 form per campus
Must be submitted by the campus principal
Due November 16, 2023 for December STAAR EOC
Due February 29, 2024 for spring administrations

Key Agreement - 1 form per "keyholder"
Must be submitted by each person who is assigned a key to the limited access secure storage location
Requires approval of campus principal
The Google form, when submitted, generates an email to the campus principal for approval. Campus principal clicks the link in the email to review and approve the form.
