All documentation for TELPAS & TELPAS Alternate is due March 13, 2026.

This form indicates all required documentation and how/where it will be submitted.

When accurate and complete, the campus coordinator will save the form in the designated folder.

Documenting "EC," "X," "M" & "N" Score Codes

A Google Sheet for Score Code Documentation was shared by email on 2.20.2025 to document any A, O, X or EC score codes.  Score codes must be thoroughly documented on the campus sheet prior to submitting in TIDE.

You must have permission from Kelly prior to assigning an "O" or "EC" score code for any student.

An "X" score code must be documented by the ARD committee prior to being submitted in TIDE.  For TELPAS Alt., document M or N score codes on this form.  An M or N  score code must be documented by an ARD committee prior to being submitted in TIDE.

Submitting Score Codes in TIDE

“S” (for score) is automatically assigned to a test submitted by a student with no further action required by the campus coordinator. All other tests require documentation that is auditable.  This includes score codes of A, O, M, N and X.

Score codes are due in TIDE as noted below:
