To request your transcript from Perryville High School, complete the Record Release Form and mail to Attention: Registrar 326 College St. Perryville, MO 63775 or fax it to the District Registrar at 573-517-0592. The processing fee is $5 per copy. For Current High School students, the first (2) transcripts are sent with no charge, after there is a $2 fee per transcript sent.
