A student registration form is one of the required documents that the school uses during the registration process every academic year to collect important and updated information about students and formalize their registration.
A pre-registration form enables parents to begin the school enrollment process in advance by providing essential student information and contact details. It assists schools in preparing for student requirements and simplifies the official registration process.
Note: This form is for returning students only.
The goal of hearing and vision screening is the identification of students who may not have normal hearing and/or vision. Early detection of hearing and vision problems is essential so students may obtain maximum benefit from their educational opportunities.
Ministry of Education provides student busing to help students who live far from schools and need transportation to and from schools. The school buses are school properties and are extensions of the schools. All school rules and regulations apply to school buses.
The need to ensure the safety and welfare of all students is a priority of the Ministry of Education. Therefore, parental consent must be obtained to allow school personnel or partner agencies to use student images. Schools must obtain parental consent allowing the school or partner agency to use student images for any purpose. A Media Release Form must be filled and signed by a parent/guardian for each event.
When a student transfers to any Palau public school, his/her official transcript and copies of other important educational information are to be furnished to the school he/she wishes to attend at least 10 working days before registration day. A transfer student will not be officially enrolled until his/her transcript and other pertinent educational information are submitted to the school. If a student transfers from one public school to another, a copy of his/her entire cumulative folder is forwarded to the new school.
Our students participate in a wide range of learning opportunities within and outside the school grounds each school year. These learning opportunities sometimes require travel outside the school and may extend outside school hours. The Ministry of Education and each of the public schools monitor these learning opportunities to identify and manage risk for all activity types. Information will be sent home about these learning opportunities and activities but permission will not be needed if you have already signed this blanket consent form.
Upon receiving information about an upcoming activity from your child’s school, you are most welcome to contact the school and remove your child from the activity.
If you choose not to do this, the school will therefore assume your child has permission, as per this blanket consent form.
Submit the forms to the school office or send them to the designated school email address.
