How to Apply

The student’s parents/guardians, at the time of registration, must provide the following:

1) Completed registration form

2) 1 Passport size photo (2" X 2")

3) Birth Certificate (original)

4) Copy of Palau Social Security Number

5) School Entrance Immunization Certificate (1st grade students only)

6) Hearing and Vision Exam

7) Complete all consent forms:

• Parent Blanket Consent Form

• Media Consent Form

• Bus Registration Form

8) Parent Teacher Association (PTA) yearly dues ~ $10.00

• For an adopted child, you must provide an affidavit.

• For Guardianship, you must provide a copy of the affidavit of guardianship.

• You must have a daytime contact number and the name of a contact person.

• For non-citizens, provide a copy of the child’s permit.

Submit school registration form and all requirements to the school office or send to the designated school email address.
