Returning Students

1. Completed Pre-Registration Form

2. School Entrance Immunization Certificate (1st grade students only)
3. Complete all consent forms:

• Parent Blanket Consent Form

• Media Consent Form

• Bus Registration Form

4. Parent Teacher Association (PTA) yearly dues ~ $10.00

• For an adopted child, you must provide an affidavit.

• For Guardianship, you must provide a copy of the affidavit of guardianship.

• You must have a daytime contact number and the name of a contact person.

• For non-citizens, provide a copy of the child’s permit.

Submit school pre-registration form and all requirements to the school office or send to the designated school email address.
