The point in this study is to collect data so that we can track progress in how we are doing with solving this problem. When filling out the survey, you should be honest, as the data collected goes straight into a chart that summarizes the answers. If we can get from an average answer of 3 times a week for each question to 5 times a week, then that would be a huge difference. Remember, a small change can make a big difference.
