ELIGIBILITY AND ENROLLMENT
Newly hired associates must enroll within their first 30 days of employment. Enrollment must be done electronically on the Kronos platform. All benefits (with the exception of 401k) become effective on the first of the month following 60 days of service.
Health Insurance (Full Time Associates): Ocean State Job Lot (OSJL) offers health insurance through UMR. Associates can choose between 3 different plan levels and can elect coverage for themselves, associate plus one, or family. Any qualified changes can be made during the annual open enrollment or within 30 days of a qualifying life event.
Dental Insurance : OSJL offers associates the opportunity to enroll in an associate contributed dental insurance program through Delta Dental of Rhode Island. Associates can choose from two (2) different plan levels for associates and their eligible dependents.
Vision Insurance: The EyeMed vision plan provides significant benefits and cost savings for your vision care. OSJL offers two (2) vision plans for associates and their eligible dependents.
Group Term Life/AD&D Insurance: OSJL offers associates a Group Life and Accidental Death and Dismemberment (AD&D). A death benefit will be paid to the designated beneficiary upon the death of the insured. AD&D insurance provides specific benefits for a covered accidental bodily injury that directly causes death of dismemberment, subject to policy limitations. All associates will be covered at 2x their annual salary. This is 100% paid by the company.
Voluntary Life/AD&D Insurance: Ocean State Job Lot offers associates Voluntary Life and AD&D coverage through Lincoln Financial. Associates can elect coverage amounts for themselves, their spouse and their children. Coverage for yourself is available in $10,000 increments with a guarantee issue amount of $350,000 and up to a maximum of the lesser of $500,000 or 5 x’s the associate’s annual salary (requires evidence of insurability).
Voluntary Accident Insurance: OSJL offers associates Voluntary Accident Insurance and AD&D coverage through Lincoln Financial. The benefit offers income protection from unexpected expenses arising from injuries sustained outside of working hours and coverage includes emergency care, fractures, dislocations, specific injuries or treatments and transitional care.
Voluntary Critical Illness : OSJL offers associates Voluntary Critical Illness insurance through Lincoln Financial. This benefit provides a payout when an associate is diagnosed with illnesses in a variety of categories, including heart complications, cancer and organ failure.
Short Term Disability (STD) (Full Time Associates): OSJL is concerned about an associate’s financial situation when an associate is out of work due to a short-term medical situation. All Full Time associates are enrolled in the Short Term Disability benefit. This benefit is 100% company paid. Associates can receive up to 60% of their salary while out on Short Term Disability.
Long Term Disability (LTD) (Full Time Associates): OSJL provides associates with a long-term disability benefit. This benefit goes into effect after following the end of Short Term Disability or after 180 days of being out of work due to non-work related disability due to illness or injury. Associates who have exhausted their STD benefit may be eligible for an LTD benefit, which provides qualified associates with 60% of basic earnings not to exceed $6,000. The benefit is 100% company paid.
