Medical/ Physical Eligibility for Athletics

Before a student can participate in tryouts, practice, or competition for a sport, he/she must have an athletic clearance from the athletic office stating the student athlete has completed the following:

• A complete physical from a Medical Doctor, Doctor of Osteopathy, or Nurse Practitioner with health history included

• Anti- Hazing Policy form

• CIF-SS Athlete Code of Ethics form

• OGSD/ RPS Code of Conduct- Parents

• OGSD/ RPS Consent to Treat

• OGSD/ RPS Football Informed Consent (Football Only)

• Concussion Information Sheet

• Heat Illness Prevention

• Sudden Cardiac Arrest

• OGSD/ RPS Opioid Fact Sheet

• OGSD/ RPS Statement of Consent

• OGSD/ RPS Student Athlete and Parent Handbook Acknowledgement

Each of these forms are available on our athletic clearance site athleticclearance.com. Blank physical forms are also available for download prior to making and attending your scheduled physical. Paper copies of the physical form are also available in the front High School office. Completed physicals can be uploaded to your student-athletes registration. 

Completed student-athlete registrations and uploaded sports physicals will be reviewed by Bridget Pfenning and Brandon Mason prior to clearance in order to ensure all required documentation is completed and provided. Please reach out to them with any questions you may have. 

Academic Eligibility for Athletics

Participation in athletics at Riverside Prep requires a student-athlete to maintain minimum academic standards to be deemed eligible to participate in interscholastic competitions. Riverside Prep and the CIF-SS standards for athletic eligibility are: 

• The student-athlete is currently enrolled in at least 20 semester credits (4 classes)

• The student-athlete passed at least 20 semester credits during the previous grading period

• The student-athlete has maintained, during the previous grading period, a minimum of 2.0 GPA on a 4.0 scale in all enrolled classes

• The student-athlete is maintaining minimum progress towards meeting the high school graduation requirements as prescribed by the OGSD
