OLG Preschool Required Forms

All required forms for OLG Preschool MUST be turned in by end of July for the upcoming school year

Required Forms

• • Emergency Card (fully completed)

  • Student Immunization Form  or submit official immunization record from physicians office

  • Health Summary (signed by Physician)

  • Individual Child Care Program Plan-this form will be provided and required if your child has a medical need, allergy, or dietary restriction.

  • Preschool Medication Authorization Form-this form is required if your child will need to receive medication at school. 

OLG Preschool Illness Policy 

The OLG Preschool follows the same Illness Policy for Kindergarten-8th grade. Please see the Illness Policy under the Illness section.

OLG Preschool Immunizations Requirements 

• Minnesota law requires children enrolled in child care, early childhood education, or school to be immunized against certain diseases, unless the child is medically or non-medically exempt. 

• All preschool students are required to submit a copy of immmuization records at the start of each school year

• Parents may file a medical exemption for specific or all immunizations, which requires a signature by a health care provider. Additionally, parents may file a non-medical exemption for specific or all immunizatins by completing the Non-medical Exemption Form signed by the child’s parent/guardian, and must have it notarized. If you plan to file an exemption for your child, this must be received by the Health Office.
