Minnesota law requires children enrolled in child care, early childhood education, or school to be immunized against certain diseases, unless the child is medically or non-medically exempt.
We request that you please provide a copy of your child’s most current immunization records to the health office on enrollment, and/or when your child has received any new or updated vaccines.
Parents may file a medical exemption, which will require a signature by a health care provider. Additionally, parents may file a non-medical exemption by completing the Non-medical Exemption form signed by the child’s parent/guardian, and must have it notarized. If you plan to file an exemption for your child, this must be received by the Health Office on your child’s enrollment or by the second Friday following the start of the school year.
Typically, school aged children are due for required vaccines around the age of 4-5 years old (prior to starting Kindergarten), and again before 7th grade. Please review your child’s immunization records and ensure they have received the state required vaccines as listed in the tables below:
*5 to 6 year olds should receive 1 dose of DTap, 1 dose of Polio, 1 dose of MMR, and 1 dose of Varicella prior to entering Kindergarten.
*Incoming 7th graders are due to receive 1 dose of Tdap, 1 dose of Meningococcal (MenACWY, Menactra) before entering 7th grade.
