Our goal for our OLG community is to provide safe, compassionate care to our students, faculty and staff. We hope to educate our students and staff about general health and wellness, prepare staff for illness related emergencies, and promote a healthy environment physically and mentally for our community. Our goal is to collaborate closely with our teachers and staff to ensure we do our best to keep our students in school, while giving them the care they need when an illness or injury may occur.
On this site, parents will find all the information, resources and requirements needed for your child’s health needs for the upcoming 2024-2025 school year. Please know that we are here to support you and your family’s needs. We are always available for questions and or concerns.
Annual Student Health Form (required for all students K-8)
Updated Immunizations Record (required for all new students, preschoolers, kindergarteners and 7th graders or if changes have been made to child's record)

Notification of all allergies, food intolerance or sensitivities
PLEASE NOTIFY THE HEALTH OFFICE IF YOUR CHILD HAS A FOOD ALLERGY OR FOOD INTOLERANCE. Please refer to the Medications section, under Emergency Medical Management and Medications for further information. Even if your child does not require medication for their allergy or intolerance, please notify the health office so we can alert Food Services.
If your child has an injury or health condition requiring special attention (eg: ice packs at school, stay in from recess or gym request) please provide a note to the health office signed by a parent and physician with the request.
If your child has an injury requiring special assistance (eg: wheelchair, crutches) please notify the health office prior to your child returning to school so we can make arrangements ahead of time.
