When medication is necessary during school hours, our intention is to insure the health and safety of your child. Each year, the following must be followed when sending in any prescription or non-prescription medication to school:
This form requires both a parent and a physician signature. All prescription and non prescription medications (includes over the counter medications like Tylenol, Advil, Benadryl) need a physician signature. This is compliant with the Edina School District medication policy.
The original pharmacy label and/or container: For non-prescription medications, the medication must be in the original container (no unlabeled plastic baggies). For prescription medications, the pharmacist can supply a labeled container, one for home and one for school. The pharmacy label must have the following information:
Student full name
Physician name
Medication time and dosage
Time and directions for administration
Current date
New medication consent form is required when:
The dosage or time of administration is changed
At the beginning of each school year
If discontinued medication is restarted. Please notify the Health Office when the medication is to be discontinued.
OLG Health office does not stock OTC medications. All medications including Tylenol, Advil, eye drops, allergy medicine, etc must be provided by a parent or guardian in the original packaging with a signed Medication Administration Authorization form by parent/guardian and a physician.
OLG Health Services is not responsible for breaking tabs in half. When there is a physician order to give half of a tablet, please talk with your pharmacist to have them pre-cut.
Medication to be administered at school will be stored in the Health Office. Exceptions are students who may carry an asthma inhaler or epipen, if they have written doctor’s orders and written parental permission. Please see Contract for Student Self Carry enclosed in the Health forms section.
At the end of the school year, all medications must be picked up in the health office by the parent/guardian or responsible adult or a signed consent form must be completed to send medications home with students. Controlled medications must be picked up by parent/guardian and will not be sent home with students. OLG will dispose of unclaimed medications following proper guidelines.
OLG Health Services does not routinely accompany students on field trips. A teacher may be responsible for administration of medication. Health Services will provide appropriate education regarding prescription and emergency medications to teachers prior to field trips. Please see Field Trip section for more detailed information.
For students requiring emergency medications for severe allergies, asthma, seizures or other, we ask that a licensed provider provide an emergency action plan for use of these medications. Please ensure they have been dated within the last year.
These Emergency Action plans will be kept in the Health Office with emergency medications and will follow students on field trips, overnight field trips, and school activities.
For your child’s privacy, only homeroom teachers will be provided with a copy of a student's emergency action plan. If you would like any other staff member or member of administration to be aware of a student's condition, please contact the health office directly for consent to release information. Please note, food services will be notified of student food allergies and food intolerances at the start of the new school year. Please notify the health office of any food related allergies or intolerances before August 28th.
*At the beginning of each school year, the OLG school nurse(s) will train all OLG faculty on the management of medical emergencies; which includes anaphylaxis and the administration of epinephrine, seizure management, and CPR/AED/First Aid.
