Kindergarten-8th Grade Health Forms

Please turn in forms to the health office or email them schoolnurse@olgschool.net 

Annual Student Health Form 

• Required for ALL STUDENTS Kindergarten-8th grade at the start of the school year

Student Immunization Form

• Required only for incoming Kindergartners, 7th graders, new students or changes to child's immunization record

Please contact the health office when your child meets the criteria for the below forms: 

Medication Administration and Authorization Form

• This form is required if your child will need to receive medication at school. 

Immunization Exemption Form

• This form is required if your child is either medically exempt or non-medically exempt (contentious objection) from required immunizations

Contract for Student Self-Carry Medication

• This form is required if your child will self carry any emergency medications (epi pen, inhaler)

Preschool Health Forms

Required forms for ALL PRESCHOOL STUDENTS to be turned in by the first day of the school year:

Health Summary (signed by physician)

Other Health related forms required for children with Allergies, Medication or other Medical needs:
