Monitoring Exited Students

OKCPS will collect monitor forms for 1st- and 2nd-year monitor students both 1st and 2nd semester for 2025-2026. Below are the slides to help you complete your Monitoring Forms.

1st Semester Monitoring Forms are due before Winter Break (December 19). Monitoring start date for your form will always be the date you are completing the Monitoring Form. The date will autopopulate.  

You can submit them via email to Stacy sebrown@okcps.org or upload them to your EL Site Compliance Folder and notify Stacy when they've been completed and uploaded.

There is no option to bulk sign Monitoring Forms. They are completed one at a time. Signature stamps are not allowed by OSDE. Forms must be either digitally signed in EduSkills or manually signed with a pen.
