Hand, Head, and Heart questions are three categories of question that you'll see on the OSSLT (and in your regular classes).
Once you learn to recognize these kinds of questions, you'll be able to figure out what kind of information the question is looking for. Once you know that, all you have to do is write out the answer!
Hand questions are looking for explicitly stated information. The answers will be stated directly in the text.
You should be able to take your hand and 'point to' the answer
Head questions are looking for implicitly stated information. Some of the answer might be in the text, but not all of it.
You need to read between the lines and use your own ideas/logic to come up with the full answer
Heart questions are looking for you to make a connection between the test/what you read and your own opinions or experience
You need to give your own opinion about a topic
