Planning a child’s transition from adolescence to adult life is one of the most important activities that will be done to prepare that child for a successful future. In Ohio, transition planning begins when a child with a disability is 14 years of age or earlier if needed. Norwayne Local schools begins to write a transition plan starting in 6th grade with all students who have qualified for special education services.
A mission of Norwayne Local Schools’ Transition Services is to facilitate integration of students with disabilities into their school and community by providing a variety of opportunities that support individual needs, interests and strengths. Through the transition planning process, families and students are introduced to resources that may be important as students move from school to post school activities. Transition services are coordinated by IEP team members to assist families and students in developing a transition plan that will move students toward their post secondary education, employment and independent living/community integration goals.

• Consultative services for future planning
• Job training internship opportunities
• Career & technical education exploration
• Post secondary programming exploration
What does a Transition Service & Support Coordinator do:
Develop a transition plan (My Plan)
Coordinate Assistance with Guardianship process.
Provide guidance and information regarding Medicaid/SSI application.
Coordinate Medicaid Waiver services such as Respite/HPC services, adaptive aids and equipment, non-medical transportation, vocational training and employment supports.
Analyze/utilize information from recent evaluations and assessments.
Provide information for an informed choice of service provider (Spring semester of final year), including tours, descriptions and breakdowns of local service providers.
Attend IEP meetings to gather information and to address concerns and questions from family members.
Make referrals to a variety of services resources, and supports to help young adults have the life they desire.
