Welcome to the NKY Health Orientation Process!
To ensure a smooth and efficient onboarding, please carefully read the information below to determine the correct orientation checklist or form you need to complete.
Are you an NKY Health Employee?
If YES (you receive pay direct-deposited by NKY Health), your orientation checklist is a digital version you will receive in an email. This email will have the subject line: "Welcome to Day 1 at NKY Health - Let's Get You Started!"
Are you a Student, Supplemental Staff, or a Volunteer?
If YES, please choose the option below that best describes your role:
Clinical Students (10+ Hours):
If you are in a clinical placement (e.g., nursing, dietetics, role transistion) or are a licensed provider with a total placement of 10 hours or more, please complete your orientation using this Google Form: NKY Health Clinical Students 10+ Hours Orientation
Students (≤10 Hours):
If you are shadowing or observing for a total of 10 hours or less, please complete your orientation using this Google Form: NKY Health Students ≤10 Checklist Google Form.
General Placement Students (10+ Hours):
If you are in a general placement (e.g., planning, environmental health, administration, harm reduction) with a total placement of 10 hours or more, you must complete an orientation at the beginning of your internship. You'll be offered either a virtual, online, or an in-person orientation. Please reach out to student.coordinator@nkyhealth.org if you have questions about your orientation.
Supplemental Staff and Volunteers:

For printable checklists, please be sure to clearly write your name at the top. For electronic forms, your identifying information will be captured digitally.
We've provided a Cheat Sheet for Sign Ons and Passwords to assist you in managing your various logins. You can print this sheet for your convenience.
If you have an alternative password management system, please feel free to use it. NKY Health cannot store or manage your passwords for you.
This step is exclusively for NKY Health employees. If you are a student, volunteer, or an employee of another agency working at NKY Health, you can skip this entire step.

Understand and Elect Your Benefits:
Use the links provided on this page to learn about the different benefits available to you (e.g., health insurance, retirement plans, etc.).
You will also find instructions on how to enroll in or elect these benefits.
Pay attention to information regarding when and how you can make changes to your benefit elections in the future.
Complete and Submit Benefits-Related Forms (Red "HR" Mark):
Some forms on your orientation checklist will be marked with a red "HR". These are benefits-related forms that require your completion and submission to Human Resources (HR).
For each "HR" marked form:
Download: Download the form to your computer.
Open with AdobeReader: Open the downloaded form using AdobeReader (this software should be pre-installed on the computer provided by NKY Health).
Complete Electronically: Fill out the form digitally within AdobeReader.
Print: Print a hard copy of the completed form.
Sign: Sign the printed form.
Scan: Scan all pages of the signed form. Each form must be scanned as a separate .PDF file.
Email to HR: Attach the scanned .PDF file(s) to an email and send it to Human Resources (hr@nkyhealth.org).
Save Originals: Keep the original signed hard copies of the forms for your own records.
For NKY Health Employees (Electronic Checklist): Your electronic orientation is organized into sections for Day 1, Week 1, and Month 1 to help you navigate your initial onboarding. You'll notice that the headings on your electronic checklist, as well as some individual checklist items, are hyperlinked. Simply click on these links to be taken directly to the corresponding page within the orientation site for detailed instructions. As you finish each task, you can mark it as "Complete," and the completion date may be automatically filled. Please follow all on-screen prompts and instructions.
For Printable Checklists (Supplemental Staff & Volunteers): You will notice some headings on your checklist that are shaded in gray. These gray headings correspond to sections within the orientation site that contain the required information and instructions for completing those tasks. After finishing an item, write your initials and the completion date in the right-hand columns. If an item doesn’t apply to your role (as indicated), you can skip it and mark it as "N/A" instead.
For Google Form Based Orientations (Students ≤ 10 Hours, Clinical Students 10+ Hours): If your orientation is completed via a Google Form, please follow the instructions provided directly within the form itself to complete your requirements.
Estimated Completion Times: Each checklist item has an estimated completion time listed to help you plan. Remember, these are just estimates; some tasks may take you more or less time.
Printing Items: If the checklist asks you to print certain items, please do so. You’re also welcome to print anything you’d like to keep for reference, regardless of your checklist format.
NKY Health Employees: Once you've finished the electronic orientation checklist, please send an email to your supervisor to let them know it's complete. In your email, be sure to cc: kim.monaco@nkyhealth.org and include a link to your completed Google Sheet checklist. Your supervisor will then review it and reply to your email to confirm its completion. This email will serve as the official record of your orientation. Remember to scan and email your supervisor any documents that are marked with a red S. There is no need to submit a physical checklist.
For Clinical Placement Students (10+ Hours), your orientation needs to be completed at least one week before your start date. Instead of a printable checklist, you'll complete your orientation via a Google Form. Once you've finished all the items on the Google Form, your responses will be submitted electronically, and no further action is required from you for your submission. If you have questions, please email student.coordinator@nkyhealth.org.
Students (≤10 Hours): If you are a student completing the Students ≤10 Google Form, your responses will be submitted electronically upon completion, and there are no additional steps for submission.
Supplemental Staff and Volunteers: When you have completed all items on the Supplemental Staff and Volunteers checklist, please give your checklist and any printed items (marked with a red S, if applicable) to your orientation supervisor.
Need Help? If you're unsure who your orientation supervisor is, please ask the person who helped you start the orientation process.
Employees, supplemental staff, and volunteers may need to meet with the Employee Health Nurse at the beginning of their assignment for TB training. If you are a nursing employee, volunteer, or consultant, you will also receive Blood Borne Pathogen training. This training will be scheduled for you.
Please be sure to bring a copy of results of your PPD/TB tests obtained within one year to this meeting.
If you are a nursing employee, volunteer, or consultant, you must also bring the following:
Your TDAP vaccination record.
Your MMR status.
Your HBV status.
