Employment Eligibility Verification (Form I-9)
All NKY Health Employees must complete page 1 of this form, provide either one document from List A or one document from each of Lists A and B, and submit both the form and copies of the document(s)Â to Human Resources (HR) before they can work at NKY Health.
Follow these steps to complete and submit the form:
Download it and fill it out electronically.
Print it.
Sign it using blue or black ink.
Attach the supporting document(s).
Scan pages 1 and 2 plus copies of the supporting document(s) into one .PDF file to your email and forward the email to HR.
Alternatively, employees may bring the paper form to their meeting with HR prior to their first day.
