Transition Services

Welcome to NKCES Transition Services Site!

On this site, you'll find transition services and supports for students as they transition out of IDEA school-based services and high school to adult life.

The site is divided into a variety of possible services and supports that a young adult might qualify for or need as they exit high school. Each section has information about possible services as well as contact information.

Whether you are a high school student, parent, teacher, guidance counselor, or school administrator, this site has information designed to help make the transition process easier. We hope you enjoy the information!

Note: this site is developed for information purposes only. NKCES is not affiliated with the services listed on this site, nor do they have any role in the admission process for any program listed. For information about any service on this site, please contact the agency directly.

New to Transition planning for students with disabilities? Start here!

We have a great starting point to dive into Transition Services - our Indicator 13 training. This free online training focuses on the mandatory/compliance requirements for supporting students with disabilities during their transition years of schooling (beginning in the 8th grade or age 14 and older and continuing through graduation or when students age out of IDEA services at age 21).

This project is focused on helping students develop self-determination skills. Includes free transition resources for students, families, and educators.
