Step 1: Contact Federated Transportation of the Bluegrass (FTSB)

FTSB is the agency that sets up transportation. There is a variety of transportation options for you to choose from. You must first have approval from FTSB, then contact the transportation agency/service of your choice.

If you have a vehicle registered to the same address as the person with a Medicaid waiver, the Kentucky Medicaid office will automatically "deny" and cancel transportation services that were approved through FTSB. The family must provide an authorized letter to the Medicaid office stating which of the following conditions apply before FTSB transportation services can be re-instated:
the drivers in the home are not able to transport the person who is Medicaid eligible due to their working hour (letter(s) must come from the driver(s)' place of employment
the driver's in the home have disabilities and are not able to drive the car (letter must come from physician)
the car in the home is not driveable - letter from driver in home
Note: these letters must be repeated every 6 months and sent to the Medicaid office to retain transportation services.
