On this page, you will find a number of Google Forms and Risk Assessments currently in use  to support all operational processes and health and safety management.

Instructions for form completion and downloading to pdf for file - for use in low, medium and medium high level activities; HIGH RISK activities as identified here to be assessed under separate cover using agreed templates as supplied by the Trust's provider of health and safety advice.

1) Enter all the information - you can cut and paste from elsewhere if needed.  NOTE - the Risk Assessment requires the (a) school name, (b) assessor name and (c) confirmation that it has been shared with the relevant staff before it will allow for printing, which will be landscape view.

2) Once you have finished everything, use the  'download to pdf' to download the information and save it locally.  If you then need to change anything, you will need to refresh the page and start again - please use F5 or refresh as you usually do.  You can also repeat this process to start a new risk assessment.

3) Next time you load the site, you will have access to a blank form and can repeat.
