SECTION A - Application Forms - for use across NCL

SECTION B - PRIMARIES - Safer Recruitment Forms

(i) STAFF - declarations - these are for the use of the Primaries: please email Joy should you wish to set up a suite for your school and this can easily be arranged.

OFFER01a - Lived or worked outside the UK - all appointed candidates to complete

OFFER02a - Criminal Records Self-declaration - all shortlisted candidates to complete

OFFER03a - ICT acceptable use agreement - all appointed candidates 

OFFER04a - DBS application instructions - all appointed candidates whose DBS certificate is older than 3 years OR who do not have a certificate

OFFER05a - Next of kin details collection - all appointed candidates

OFFER06a - New starter - personal information and bank details - all appointed candidates

REF02a - Staff - reference request - all staff who will be working at the Trust's primary schools

(iii) CONSULTANTS - safer recruitment and vetting checks and onboarding documentation - all

CON01a - Primaries - Consultants and Contractors - regulated and unregulated activity - to be completed by the supplier (ie. the employer) of the consultant/contractor, to confirm the checks have been undertaken

REF01a - Consultants - reference request - all consultants who will be working with the Trust

CON02a - Primaries - Consultants and volunteers >5 days working at schools - onboarding documentation for review
