The school nurse or health aide is available to help with health concerns.
The school health office can be reached by calling 651.464.8989. A licensed school nurse and supervised health aide provide health services, including health counseling, education, and screenings for students.
Notes for exclusion from participation in Physical Education class should be brought to the Health Office.
School-Administration of Medication
If your child will need ANY medication during the school-day, prescription or non-prescription, they must have a Medication Dispense form on file with the school.
Self -Carry and Administration of Acetaminophen or Ibuprofen (Upper School only)
If you want to allow your student to self carry and take acetaminophen or ibuprofen at school, we must have your permission on file. - Self -Carry and Administration of Nonprescription Pain Medication . This form must be updated annually.
The medication must be brought to school in a properly labeled bottle and not expired.
The student must not share the medication with anyone else.
Minnesota law requires that all students get vaccinated against certain diseases or have a legal exemption on file to enroll in school. All immunizations must be up-to-date before the 1st day of school in September.
Students entering 7th grade need to have documentation or a legal exemption for a Tdap and a Meningococcal vaccine.
Students entering 12th grade need to have documentation or a legal exemption for the booster dose of meningococcal vaccine.
All exemptions must be updated at the time of new requirements even if past exemptions have been completed.
These forms can be found on MDH website (English) (Spanish). Medical and Non-Medical exemptions must be updated at each grade level when there are new requirements (7th and 12th).

Local pharmacies (Cub, Walgreens, etc)
