Booking an Appointment
You can use the below form to get in touch and let us know your preferred appointment dates. After you have submitted this form, one of our team will review this and send you a confirmation email with your appointment time/date & details of what to do next. Please read the information on the form below and your confirmation email carefully.

Collegiate Campus
You can find us at Hallam Active: Pearson Building - Sport Hallam, Broomgrove Rd, Broomhall, Sheffield S10 2BP (the postcode alone may direct you to the opposite end of the building, please use Broomhall Road entrance)
Please note parking around the area is pay and display and limited at peak hours.
City Campus
You can find us in the Surrey Building: Pond Street, S1 2PP

Upon booking you will be sent a confirmation email, with a link to pay for the appointment. If you have already registered your online account, and have been given a PIN, you can use the link below to complete your payment.
Cancellation policy
We understand that sometimes plans change, or there are reasons you cannot attend your booked appointment, however if you do need to amend or cancel your appointment we ask that you inform our reception team at least 24 hours before your appointment, so that we may offer this to someone on our wait lists. Failure to give 24 hours notice will result in the following:
Paying clients (students / Staff / External): The appointment fee will be held on your account and payment requested. You will not be able to book any further services until your fee has been paid in full.
Non-paying clients (Team Hallam Ultimate / PASP / TASS): The session/s will be removed from your yearly allocated allowance.

Privacy Notice
Due to government and physiotherapy governing body guidance, all initial contacts must be screened for COVID and risk assessed for suitability for a face to face appointment. The booking form will ask you questions that will allow us to perform this in an efficient a manner as is possible. It is therefore important you complete all sections of this form, otherwise we will be unable to process your request for an appointment. By submitting this form you agree to SHU processing and handling your data.
All in person appointments requests will be provisional until one of the therapy team have reviewed the information you have submitted below and risk assessed in line with government guidance.

If we need to share your information i.e. with your G.P. or another medical provider as part of your ongoing care, we will only do so with your express permission on request to the physiotherapy clinic by yourself. Your information will not be shared with any third parties other than those previously mentioned and on your request only.
All medical data records will be held for a maximum of 8 years if over 18 years of age since the last record of treatment, and 8 years after your 18th birthday, up to the age of 25, if under 18 years of age during treatment. See here for further information
