-Original Copy of Honorable Dismissal

-Original Copy of Marriage Certificate (PSA/NSO COPY) for Married Female Student)

-1 pc. 2x2 Colored ID picture

NOTE: NO ENROLMENT FOR STUDENTS WITH INCOMPLETE REQUIREMENTS

OLD STUDENTS MUST present ANY of the following:

-Previous Registration Form

-MPC ID or ANY Valid ID
