Preparing for Postsecondary Success
Monroe County Special Education Department supports students with disabilities as they prepare for adult life. Transition planning begins no later than age 14 and is personalized to the student's strengths, preferences, and interests.

Individual Learning Plans (ILPs) aligned to strengths and goals
Career Pathways and CTE credentialing
Job readiness and supported employment
Connection to agencies such as Voc Rehab, Department for the Blind, and other adult service providers

Our transition team also partners with families and local agencies to ensure every student has a plan for success beyond high school.
