Upon the demise of direct family members as listed in the Form ESWS/1.A, the ESWS shall pay the members “Semso” as per the following entitlements:
It shall be the responsibility of members to report changes in their legal marital status and updates of their children promptly. These reports shall be submitted to the Manager through the respective DEO/Head of Institute/AFD as the case may be.
Members who have the same parents and are also enrolled in the same scheme may submit separate applications for the same “Semso” case according to their individual eligibility.
Applications for “semso” (Form ESWS/2) shall be submitted to the ESWS Manager as soon as possible and not later than 12 months after the demise of a direct family member. Applications received thereafter shall not be processed. “Semso” shall be disbursed upon being notified through the DEO/Head of Institute concerned of the demise of the direct family member/s of the member and production of any one of the following documents:
The ESWS shall disbursed payments not later than 15 days after the receipt of complete documentation.
