Requests to Store & Administer Medications for SY 2025-2026:
Parents who anticipate storing medication or epipens in Mililani Middle School Health Room for Sy 2025-2026 must fill out DOE form SHA36, Revised 1/2024 by June 9, 2025 and turn it into the MMS Health room for processing. Forms will be reviewed and processed by the Public Health Nurse (PHN) over the summer break. Upon approval to store medication, parents will be notified to bring their medications into the Health Room for storage. Medications will not be stored in the MMS Health room unless approved by the PHN. It takes up to 30 days to process medication forms. Form SHA 36 may be picked up at the MMS Office or Health Room.
Message Regarding e-bikes
We were recently made aware by Honolulu Police Department officers that, by law, an individual must be 15 years of age to operate an electric bike (e-bike). As such, effective immediately, students who are not 15 years of age or older will not be allowed to bring an e-bike to school campus. Please share this important information with your child. We appreciate your help and support with this.
