A comprehensive evaluation that includes transition assessments must occur before the end of grade 9. Per MDE, transition assessments cannot “stand alone." If the evaluation is being planned during a student's 7th or 8th grade years, transition assessments should be added to the evaluation plan. Transition evaluations must include at least two assessments, one of which must be a formal evaluation tool. After transition is evaluated, all IEPs must be written as transition IEPs with all of the transition sections address and goals in at least the areas of Post Secondary Education and Training and Employment.
A transition evaluation must address all areas of transition including: Post-Secondary Education and Training, Employment, and Independent Living (Recreation & Leisure, Home Living, & Community Participation). Transition assessment must not only identify needs but must also identify the student’s strengths, preferences, and interests.
