Other Health Disabilities (OHD) includes a wide range of chronic or acute health conditions, including Attention Deficit/Hyperactivity Disorder (AD/HD), that can range from mild to severe. Medications, treatments, therapies, and repeated hospitalizations can affect a student’s ability to learn and function at school. A student with such a condition may be considered for special education under the OHD category.

Gathers data on organizational, work, and motor skills from teacher ratings of numerous behaviors based on frequency of observation. Forms for Infant/Toddler, Pre-K/Kindergaten, Elementary, Middle/High School and includes a report writing sample.
Observation Forms
For eligibility in Other Health Disabilities (OHD), documentation of at least one observation in the classroom or other learning environments is necessary. Focus on how the disability affects the learning process.

Momentary Time Sampling Observation Form - EditableMomentary Time Sampling Observation Form
The observer records whether a behavior did or did not occur at the end of the interval. Momentary Time Sampling is an ideal observation method for a variety of behaviors where it is difficult to tell exactly when the behavior begins or when it ends, and behaviors that occur at such a high rate that it is difficult to keep a count. It is commonly used to track on-task vs. off-task behavior.
Systematic Observation Rating Form
The observer makes ratings for various behaviors under the categories of attention, time management/planning, motor/access, and organization/materials.
