HOW TO SUBMIT SPORTS PHYSICALS

• During the school year, the pre-participation eligibility form can be dropped off in the health office by your child. Please do NOT leave forms in the mailbox during the school year UNLESS YOU SPECIFICALLY REACH OUT AND TELL US YOU HAVE. We do not routinely check the mailbox during the school year.

• During the summer months, the pre-participation eligibility form can be submitted in two ways:

• by mail:

  • Manalapan Englishtown Middle School, 155 Millhurst Road, Manalapan, Attn: School Nurse

• dropped in the indicated drop box at the main entrance of the school:

  • If you choose to use the drop box, please put the forms in a plastic zip top bag to protect from the elements. 

DO NOT email sports forms. They cannot be printed in the volume we receive

and will not be accepted.

GENERAL MEDICAL INFORMATION FOR MEMS SPORTS

All physical forms and other medical forms are available on the district nurse website. Scroll down and click on the link for the "NJDOE Student Athletic Forms Letter" (linked here or directions on how to find it online below). 

All of the required forms are hyperlinked in the above letter. Please make sure you print all of the forms to bring to your provider. It is unlikely they have the new required State forms and the older “Sports Physical Packet” will not be accepted:

• Preparticipation Physical Evaluation History Form 

• Preparticipation Physical Evaluation Physical Examination Form 

• Preparticipation Physical Evaluation Athletes with Disabilities Form: Supplement to the Athlete History

• Preparticipation Physical Evaluation Medical Eligibility Form (required to be turned into the nurse's office)

These forms will be available in a "packet" from the health office during the school year.

IMPORTANT: If your rising 7th or 8th grader has a valid sports physical from this past school year on file and it will not be expired at the start of the season, it will be “grandfathered in”, per the State of NJ. If you are not sure if your child’s sports physical is still valid, please reach out to the health office to check the status of that physical as soon as possible so you have time to make an appointment with your medical provider if necessary. 

Health History Update Questionnaire and Consent Forms  will be provided at the mandatory pre-season meetings (MEMS).  The state required sign-offs (which were previously included with the medical packet) will be paperless in the parent portal for signatures at the start of the school year.

Please make sure you are printing the aforementioned sports forms and not a "Universal Child Health Record" (formerly "entrance physical"). The entrance physical is not the same and cannot be accepted as a sports physical. 

Sports physicals are valid for 365 days from the date of exam, not from the date they are submitted, per the State of New Jersey. You can also print any necessary medical forms / orders from the District Nurse website (asthma, epipens, diabetes, seizures, etc).
