DO NOT BRING MEDICATIONS / MEDICATION FORMS TO SCHOOL OVER THE SUMMER. THEY CAN ONLY BE ACCEPTED BY THE NURSE IN PERSON. Information about Med Drop Off will be sent in August.
Incomplete packets / forms will be returned. Please contact Nurse Genna if you have any specific questions.
All necessary medical forms, including forms for medication, allergy packets, asthma action plans, seizure action plans, diabetes action plans and sports packets can be found on the
district nurse website.

Print out all forms for each required packet.
All medications, including over the counter meds like acetaminophen (tylenol) and ibuprofen (advil / motrin), eye / ear drops, and any other medication required during the school day for a non-life threatening condition require completed medication orders.
All orders must be dated on or after 7/1/2026 to be valid for the upcoming school year.
Once the forms / medication has been received, I will send home an Individualized Health Plan (IHP) to be signed and returned.
