Transcripts sent to Educational Institutions need to be sent directly from Parchment (our transcript partner). Students must create an account through Parchment and request that their transcripts be sent. The School Counseling Office approves the Parchment request once the written request has been submitted. The student is not able to handle these documents. There is a $5.00 processing fee for Alumni.
The School Counseling Office Registrar will approve the student’s transcript after the student has completed their on-line college application and takes the following steps:
Students/Alumni need to print and complete a MHS Transcript Request linked here, or paper copies are available in the School Counseling office. This form requires the parent’s signature if the student is under 18. One form is required per transcript unless transcripts are requested at the same time.
Students must create an account with Parchment. Students must request that their transcripts be sent to their college of choice through Parchment. The counseling office must approve your transcript, but we can only do this if we have the signed transcript request form.
