To complete a chapter service project, follow these steps:

Step 2: Contact the individual(s) listed as the contact person(s) on the sign-up.
Step 3: Download & complete service verification form and Parent Permission and Student Acknowledgement of Transportation Agreement Form (if service occurs off campus).
Service Verification Form
This form is for chapter & individual service projects. Be sure to get this filled out AND get a picture of yourself in action. Submit the photo to the indicated Google Classroom assignment. Once you have completed the service, bring the signed form to Mrs. Juster in room 219.

Parent Permission & Student Acknowledgement of Transportation Agreement Form
Be sure to get this completed BEFORE you do your Chapter Service Project. AFTER you have completed your service, hand-in this form along with your service verification form.
