The Support Departments work alongside classroom teachers and parents to help students be successful at school. Areas of support include ELL (English Language Learning), academic, social/emotional, counselling, health and well-being counselling.
Student Support Service Referral Form ***Please read the information below before referring a student.
Please note the following prior to referring the student to the Student Intervention Team (SIT):
1. Prior to referring the student, we ask you to provide a differentiated programme of learning in support of the student’s cognitive/academic, language and/or social and emotional needs.
2. In the process of ascertaining the needs of the student, you must keep the student’s family well informed through meetings and correspondence.
3. The student’s needs must also have been discussed with the Student Support Services staff members allocated to your year level. Student Support Services staff members will attend at least one of the year level team meetings per week, and the Student Support Services Leader of Learning meets on a fortnightly rotation, (the Briefing Meeting) with each year level. During these consultations and opportunities for information sharing, please raise the student’s needs and obtain advice.
4. Should this information sharing and programme of support be insufficient to assist the student, then please refer the student to the Student Intervention Team. Simultaneously, inform the family that you are making the referral. This should be done via telephone call or a meeting. Once the parent/s has been informed, please adjust this template and send it via email to confirm your discussion with them.
5. The Student Support Services Leader of Learning will inform you when the referral has been considered by the Student Intervention Team. Sometimes you may be asked for additional information, or invited to attend a meeting of the Student Intervention Team to further explain your concerns about the student.
6. Once the referral has been approved for further investigation, a team member of the Student Support Services Department will be appointed to continue the referral process; assessments will be undertaken by that team member and reported back to you and the Student Intervention Team.
7. If the assessment data suggests that the referred student would benefit from further assistance, then the Student Intervention Team will approve a service to the student. You will be informed by the Student Support Services Leader of Learning of the Student Intervention Team’s decision about the approved level of service.
8. In the case of students in Years 2 - 4 with very limited English language proficiency, the EAL Teacher may have already started a programme of language support for the student over the course of the referral process.
