Transition planning services are dedicated to strategically coordinating a student's transition from the school environment to meaningful post-school activities. Transition planning begins when a student reaches 14 years of age and is focused on establishing measurable, outcome-based post-secondary goals.
This process is built upon the collaboration of the student, family, and LHISD staff, in a partnership designed to offer support and development not only for academic achievements but also for acquiring the essential life, vocational, and independent living skills critical for the student's long-term future.
The Texas Education Agency Transition and Employment Guide provides information on statewide services and programs that assist students enrolled in special education programs and their families with the process of transitioning to life outside of the public school system.
